[Dcmlib] Visual Studio 7 .NET 2003 link errors

Many thanks to Jean Michel and Mathieu!

Everything now compiles, links, and tests successfully.
I have uploaded my Experimental build to your Dart server.

Also note that in order to compile the vtkGdcm tests with VTK 4.4.2, I 
have updated the vtkTesting class (as well as then having to update 
vtkSmartPointer ... and make some vtkTimerLog mods ... but I digress).
