[Dcmlib] Minwg32 users / gdcm 1.0.x

Hello,

	I finally got the motivation to go through the mingw32 problem and 
shared lib. I believe that both gdcm-cvs and gdcm-1.0.x branch are fixed 
now. (tag is: Version1_0)

Anyone interested please update your cvs branch and let me know of any 
problem

Ps: I fixed a serious problem in gdcm 1.0.x branch this morning, anyone 
using gdcm 1.x should update to this revision (gdcmDebug.h 1.33.2.1), 
only affect gdcm compiled in release mode (none of the dashboad machine 
is doing it...)
