[Dcmlib] MPEG2 alpha

Hello,

	After hacking like crazy this so called 'library' I think I have a 
working prototype. EXTREMELY alpha, but in case you have a small MPEG2 
sample I believe it should work.

	I try running it on the ETIAM sample but it just blows up, whereas if I 
can tell him to use a smaller MPEG2 sample(*) it works.

	I am not turning the compilation on in any dashboard until I get 
JPEG2000 cleaned up. But feel free to try it out. Reports welcome.
