[Dcmlib] Private dictionaries

Over the week end I decided to convert some dicom3tools private 
dictionary into GDCM' XML format:

If you open the link in your browser you should see them as an HTML 
table. For now it is missing the important 'Owner' information, but it 
should be fixed this week (I need to fix the XSL transformation).
