[Dcmlib] can't compile with choosing VTK_DIR

I think it solves the problem. Another compile error is in
Test/ShowDicom.cxx. I think vtkTesting can not be supported
in VTK4.21.  maybe it is in the interim version of VTK. However, I think it
has nothing  with the vtkGdcm....
